Once you click the button above, it will open the template. To save your own version of the template, click the three dots in the top right corner and then select “Make a copy.”

This will allow you to save the form to your own Google Drive and customize it with your town’s information. At this point you can rename the survey and also edit/add questions to fit specific needs in your town.

To send out the survey, click the SEND button. This will allow you to enter specific emails, or generate a link that can be shared for all town members to access. Then you’re ready to start collecting results!
